What you will do

• Conduct eligibility screenings via phone, email, and community outreach.
• Guide prospective participants through the enrollment process and documentation.
• Maintain accurate participant records and data integrity within CRM systems.
• Meet individual and team enrollment targets through consistent follow-up.
• Coordinate handoffs to care management and service delivery teams.
• Build relationships with community organizations and healthcare providers.

Requirements

• 1–3 years of experience in enrollment, healthcare outreach, patient access, or social services.
• Must be based in The Bronx.
• Strong interpersonal skills for engaging diverse populations.
• Proficiency with CRM systems and Microsoft Office.
• Ability to manage multiple priorities and meet performance goals.
• High school diploma or equivalent required.
