What you will do

• Design and coordinate cross-functional outreach campaigns to bundle clinical care gaps.
• Monitor enterprise quality scores and manage milestones for NCQA re-accreditation and audits.
• Partner with the Data team to reconcile payer data and maintain a clean source of truth.
• Collaborate with clinical stakeholders to optimize EHR documentation logic for automated metric capture.
• Define intervention logic and target rules for rising-risk populations.
• Synthesize complex data into performance narratives for MBRs, Board decks, and payer meetings.

Requirements

• 5-7 years of experience in healthcare operations, population health, or quality improvement.
• Strong command of healthcare quality metrics, HEDIS measures, and value-based care frameworks.
• Proven track record of influencing cross-functional teams without direct line-management authority.
• Proficiency with data tools like Looker and EHR systems like Elation.
• Meticulous attention to detail regarding internal documentation and regulatory tracking.
• Must be authorized to work in the United States.

Nice to have

• Active RN license with clinical and operational experience.
• Certified Professional in Healthcare Quality (CPHQ) certification.
• Experience within a Health Plan environment.
• PMP certification or formal training in managing complex timelines.
