Location: Remote (Must have active licensure in NJ, NC, FL, or NY)
Salary: Starts at $115,000 + annual bonus

is a tech-enabled, pediatrician-led medical group providing virtual-first and in-home care for children with complex health needs.
What you will do
- Manage a panel of complex pediatric patients, overseeing care plans and ensuring comprehensive coordination.
- Collaborate with a multidisciplinary team including pediatricians, nurses, pharmacists, and social workers.
- Deliver high-quality virtual care through enrollment visits, acute escalations, and post-hospital follow-ups.
- Educate and support parents to navigate their child's health journey with confidence.
- Coordinate prescriptions and streamline referrals to bridge care gaps and remove barriers to care.
Requirements
- Minimum 5 years of experience with a pediatric population.
- Active licensure in NJ, NC, FL, or NY is required.
- Completion of an accredited NP or PA-C program with unrestricted state license and board certification.
- Proven experience managing children with complex medical needs.
- Bilingual Spanish is strongly preferred.
Nice to have
- Experience in Community Based Health or Complex Care.
- Knowledge of Medicaid and Community health systems.
- Prior experience in virtual care delivery.
- Additional licensure in GA or DC.
