Salary: Starts at $115,000 + annual bonus

What you will do

• Manage a panel of complex pediatric patients, overseeing care plans and ensuring comprehensive coordination.
• Collaborate with a multidisciplinary team including pediatricians, nurses, pharmacists, and social workers.
• Deliver high-quality virtual care through enrollment visits, acute escalations, and post-hospital follow-ups.
• Educate and support parents to navigate their child's health journey with confidence.
• Coordinate prescriptions and streamline referrals to bridge care gaps and remove barriers to care.

Requirements

• Minimum 5 years of experience with a pediatric population.
• Active licensure in NJ, NC, FL, or NY is required.
• Completion of an accredited NP or PA-C program with unrestricted state license and board certification.
• Proven experience managing children with complex medical needs.
• Bilingual Spanish is strongly preferred.

Nice to have

• Experience in Community Based Health or Complex Care.
• Knowledge of Medicaid and Community health systems.
• Prior experience in virtual care delivery.
• Additional licensure in GA or DC.
