What you will do

• Interface directly with patients, providers, and families to educate them on the value of palliative care.
• Coordinate and schedule palliative care appointments for patients in need of the program.
• Manage clinical files, enter notes into systems, and close out completed cases daily.
• Review patient follow-ups to ensure high-quality care delivery from the provider network.
• Balance time between clinical triage and high-volume patient calls within a contact center environment.

Requirements

• Minimum 3+ years of experience as an LPN.
• Minimum 3+ years of experience reading and updating clinical notes.
• Minimum 1+ years of relevant experience in a customer service call center.
• Ability to handle calls from patients, caregivers, and providers with empathy.
• Strong written and verbal communication skills with high attention to detail.
• Secure Wi-Fi connectivity and comfort working in a dynamic startup environment.

• Spanish-speaking proficiency.
