What you will do

• Conduct telephonic clinical assessments, pain assessments, and medication reconciliation.
• Coordinate care transitions, including discharge planning with hospital case managers and follow-up care with providers.
• Educate members and caregivers about diagnoses and treatment plans to support higher-acuity oncology needs.
• Monitor member progress and establish targeted support plans during case conferences.
• Partner with non-clinical teams to address social determinants of health, such as transportation and food resources.
• Handle urgent clinical escalations and provide clinical consult support.

Requirements

• Active, unrestricted compact Registered Nurse (RN) license (willingness to obtain additional state licenses).
• Degree in Nursing (ADN or BSN required; BSN preferred).
• Minimum 5 years of nursing experience, including 3 years of high-acuity adult oncology experience.
• Certification as OCN, AOCN, AOCNS, or CCM (required at hire or commitment to obtain within 2 years).
• Must be located within the lower 48 United States due to PHI access restrictions.
• Experience with remote work and proficiency with technology (Slack, Google Suite, EHR).

Nice to have

• Fluency in Spanish, both written and verbal.
