Complex Oncology Nurse Navigator (Medtech)

Location: Remote or hybrid, but must be located within the lower 48 United States. Hybrid work is available from the Nashville office.

Salary: $46.35/hour.

What you will do

• Conduct telephonic clinical assessments, including pain assessments and medication reconciliation, and monitor changes in member health.
• Coordinate care transitions, admissions, discharge plans, follow-up care, referrals, medications, and appointments.
• Support higher-acuity oncology members through urgent clinical escalations, virtual home safety evaluations, and targeted care plans.
• Collaborate with hospital case managers, providers, payers, healthcare teams, and non-clinical care staff to address clinical and social needs.
• Provide education to members and caregivers and build trusting relationships through culturally competent, empathetic communication.
• Share member and provider feedback with clinical leadership and contribute to administrative and service-improvement projects.

Requirements

• Active, unrestricted compact Registered Nurse license required, with willingness to obtain additional state licenses as needed.
• ADN or BSN in Nursing required; BSN preferred.
• At least 5 years of nursing experience, including 3 years of high-acuity adult oncology experience.
• OCN certification is required at hire, or must be obtained within 2 years of hire; AOCN, AOCNS, or CCM certification is also accepted.
• Experience working remotely and willingness to learn new technology, including electronic health records, Google Suite, Slack, and video-chat tools.
• Must be located within the lower 48 United States due to contractual PHI-access limitations.
