What you will do

• Implement products and execute ACO initiatives in coordination with Market Leadership.
• Manage wellness visits, transitions of care, and active management to ensure practices meet transformation goals.
• Train physicians and office staff on Value Based Care Curriculum and clinical technologies.
• Build and nurture relationships with healthcare executives to improve quality and financial performance.
• Act as the primary point of contact for practices, relaying feedback to HQ for process improvement.

Requirements

• Must be a resident of Florida.
• Graduate of an accredited college or 4-6 years of relevant medical practice management experience.
• 3+ years of experience working with Electronic Health Records (EHR).
• Ability to travel up to 40% within the market (est. 1-2x a week onsite).
• Excellent communication, organizational, and project management skills.

• Familiarity with process mapping and work-flow analysis tools.
• Strong computer skills and ability to quickly learn new software applications.
