What you will do

• Manage an active caseload of members with cancer diagnoses from initial diagnosis through survivorship.
• Conduct comprehensive oncology intake assessments to establish member needs and care priorities.
• Educate members and families on diagnosis and treatment options using NCCN and ONCC-aligned guidance.
• Coordinate multidisciplinary care by facilitating communication between members, treating providers, and the internal care team.
• Identify and mitigate psychosocial, financial, and logistical barriers to care.
• Maintain accurate and timely clinical documentation within the care management platform.

Requirements

• Bachelor's degree in Nursing (BSN).
• Active RN license in a compact state with the ability to be licensed in all 50 states.
• Minimum 3 years of oncology nursing experience.
• Active ONCC certification in Oncology Nursing (OCN).
• Fluent bilingual proficiency in English and Spanish for clinical conversations.
• Must be based in the Dallas area.
