What you will do

• Conduct remote telephonic and video nursing assessments for heart failure, comorbid conditions, and psychosocial factors.
• Provide tailored clinical education on disease processes, medications, symptom monitoring, and lifestyle counseling.
• Develop and evaluate personalized care plans with SMART goals in partnership with members and caregivers.
• Collaborate with a multidisciplinary team of nurse practitioners, pharmacists, and social workers to coordinate care.
• Coordinate care with PCP partners and other clinical teams within the Devoted Medical Group.
• Provide feedback and potential solutions to improve operational processes and software tools for the emerging Heart Center of Excellence.

Requirements

• Active multi-state Compact RN license.
• Minimum 4 years of RN experience, with at least 2 years in a non-ICU cardiac focused setting (outpatient preferred).
• Must be based in the Mountain (MT) or Pacific (PT) time zone.
• Proficiency with EMRs, messaging platforms, online portals, and AI chat tools.
• Ability to multi-task, including listening, talking, and typing simultaneously.
• A secure and private workspace with a strong and reliable internet connection.

Nice to have

• Telehealth or remote work experience.
• Bilingual proficiency in Spanish.
• Experience working in the Medicare Advantage landscape.
