What you will do

• Conduct remote nursing assessments for heart failure, comorbid conditions, and medications via phone and video visits.
• Provide comprehensive clinical education on disease processes, symptom monitoring, and lifestyle counseling.
• Develop and evaluate SMART goal-based care plans in partnership with members and caregivers.
• Collaborate with an interdisciplinary team of nurse practitioners, physicians, pharmacists, and social workers.
• Coordinate with PCP partners and other clinical teams to deliver evidence-based healthcare.
• Provide feedback to improve operational processes and software tools for the emerging Heart Center of Excellence.

Requirements

• Unrestricted, compact RN license.
• Bilingual proficiency in English and Spanish for conducting patient assessments.
• Minimum 4 years of RN experience, with at least 2 years in a non-ICU cardiac focused setting.
• Based in Mountain (MT) or Pacific (PT) time zone; availability to work 8-5 MT.
• High proficiency with technology, including EMRs, messaging platforms, and AI chat.
• Secure, private workspace with a strong and reliable internet connection.
