TL;DR
Provider Success Manager (Healthcare): Establish relationships with healthcare providers and drive adoption, expansion, and success of Counterpart Assistant with an accent on Medicare, primary care operations, and value-based care. Location: Must reside in West Virginia, United States and be willing to travel to practices up to 5 days a week (75% travel).
Salary: $129,000 - $150,000 USD

is building an AI-enabled primary care tool, Counterpart Assistant, to support earlier diagnosis and management of chronic conditions in value-based care. A subsidiary of Clover Health.
What you will do
- Own and manage relationships with primary care provider partners as the primary point of contact, building trusted consultative partnerships to drive adoption of Counterpart Assistant.
- Support provider recruitment, onboarding, and training as a product SME, focusing on in-office workflows and effective platform adoption across clinical and non-clinical staff.
- Optimize provider success and retention through structured engagement plans to maximize platform use and achieve measurable value-based care outcomes.
- Lead data-driven decision-making by analyzing performance metrics, identifying trends, opportunities, and risks, and driving interventions to improve outcomes and utilization.
- Influence adoption of best practices in various PCP practices and collaborate cross-functionally with sales, product, and operations teams based on provider feedback.
Requirements
- 5-7+ years in provider engagement, healthcare account management, or value-based care consulting with proven track record in adoption and retention.
- Proven ability to build relationships with clinicians and staff, drive operational change, and translate data into actionable metrics for clinical and financial outcomes.
- Deep understanding of primary care/ambulatory workflows and success implementing or improving clinical processes.
- Knowledge of EHRs (ideally primary care), medical billing, reimbursement, Medicare Advantage, and risk-based payments.
- Strong project management skills to prioritize initiatives and drive outcomes.
- Willing to travel up to 75% of the time.
Nice to have
- Understanding of value-based care models, population health, or quality improvement.
- Experience implementing value-based care tools/technology in primary care.
- Knowledge of Medicare insurance, wellness visits, coding, Stars, HEDIS.
