What you will do

• Conduct virtual primary care visits, including comprehensive assessments and acute care interventions via telehealth.
• Diagnose and manage chronic conditions such as hypertension, diabetes, and COPD using individualized care plans.
• Order and interpret lab tests and imaging, coordinating necessary follow-up care and specialist referrals.
• Manage medication prescriptions, titrations, and refills while ensuring patient safety.
• Provide health coaching and lifestyle guidance to empower patients in managing their own health.
• Collaborate with multidisciplinary teams and provide feedback to improve the electronic health platform.

Requirements

• Active APRN or PA license in good standing in one of the 29 supported states or ability to obtain licensure in multiple states within 90 days.
• Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
• Minimum 2 years of experience in primary care, urgent care, or internal medicine.
• Active BLS certification at time of hire.
• Flexibility to align shifts with standard business hours (9:00 AM – 5:00 PM) in the patient’s local time zone.

Nice to have

• Experience managing chronic conditions and preventive care specifically via telehealth.
• Familiarity with value-based care models, population health management, and quality metrics (HEDIS, STAR ratings).
• Comfort with digital documentation and telemedicine EHR platforms.
