Advanced Practice Provider (Palliative Care): Providing compassionate, patient-centered goals of care counseling and symptom management for members with life-limiting illnesses via a virtual program with an accent on longitudinal panel management and hospice transitions. Location: Remote (USA). Must hold an unrestricted advanced practice license in at least one of the following states: FL, TX, AZ, OH, CO, OR, TN, IL, AL, HI, NC, SC, PA.

Salary: $130,000 – $170,000 total compensation

What you will do

• Manage a longitudinal panel of 40-50 high-risk patients via telephonic and video encounters.
• Perform initial assessments and develop Palliative Plans of Care for new program members.
• Provide consultative visits focusing on advanced symptom management, goals of care, and disease education.
• Evaluate hospital admissions to identify root causes and implement solutions to maintain readmission rates ≤ 15%.
• Collaborate with interdepartmental clinical teams and leadership to coordinate seamless patient care.
• Provide on-call weekend and holiday coverage on a rotating basis.

Requirements

• Unrestricted advanced practice license in FL, TX, AZ, OH, CO, OR, TN, IL, AL, HI, NC, SC, or PA.
• 5+ years in advanced practice, including at least 3 years in Palliative care.
• ACHPN® credential, or willingness to obtain it within the first six months of employment.
• Ability to flex between US time zones.
• Comfort working in a fast-paced, virtual startup environment with various technology tools (Slack, Google Office, EHR).

Nice to have

• ELNEC® or EPEC® training.
• Prior experience in ICU, oncology, or geriatrics nursing.
• Prior home-based palliative care or program development experience.
• Experience in interprofessional team leadership.
