What you will do

• Conduct telephonic and video visits with patients and caregivers to build trust and strong relationships
• Collaborate interdisciplinary with clinicians, nurses, and care coordinators for cohesive patient-centered care
• Perform psychosocial assessments, create personalized care plans, provide emotional and caregiver support
• Lead goals of care conversations, advanced care planning, long-term care planning, and hospice transitions
• Manage family dynamics, facilitate family meetings, handle behavioral health referrals, and investigate abuse/neglect concerns
• Document interactions, participate in team huddles, and coordinate with local stakeholders

Requirements

• Master’s degree in social work and active license
• Minimum 2 years experience in palliative or hospice, proficient in goals of care conversations, advanced care planning, grief support
• Certified hospice and palliative social work credential or obtain within one year
• Comfortable with technology, fast-paced work-from-home environment
• Strong interpersonal and communication skills
• Comfortable with video and phone patient interactions

Nice to have

• Bilingual in English and Creole, Spanish, or Vietnamese
• Experience in health insurance, especially Medicare Advantage
• Experience in geriatrics
