Behavioral Health Care Coordinator (Pediatrics): Provide case management services for high-risk children with special healthcare needs, working alongside families, pediatricians, and care teams with an accent on behavioral, social, emotional, and systemic support. Focus on conducting comprehensive assessments, tracking care plans, crisis de-escalation, and facilitating referrals to resources and external services.

Location: Remote (Monday-Friday, 8:00am-5:00pm ET or 10:30am-7:00pm ET; 10% travel for training)

Salary: $70,000 - $77,000 + bonus

What you will do

  • Monitor high-risk pediatric patients post-hospital discharge, ensuring follow-up and resource support.
  • Conduct assessments of behavioral, social, emotional needs; provide referrals and coordinated communication.
  • Track family needs daily, evaluate and modify care plans based on progress.
  • Maintain clinical documentation in Athena and internal systems.
  • Deliver interventions for crises, behavioral issues, and family conflicts; escalate high-risk cases.
  • Facilitate navigation of mental health systems and research external services.
  • Provide psychoeducation on mental health and collaborate with external teams like schools and specialists.

Requirements

  • Masters’ degree in social work or related field required
  • Minimum 3-5 years post-graduate experience in healthcare social work/case management in behavioral health
  • Experience with pediatric population and family systems
  • Proficiency in motivational interviewing, goal setting, needs assessment
  • Knowledge of crisis prevention/intervention and telehealth services
  • Ability to work afternoons/evenings and 10% travel

Nice to have

  • Provisional licenses (LMSW, PLPC, LAMFT)
  • Bilingual Spanish
