TL;DR

Bilingual (Spanish) Advanced Practice Provider (NP or PA) (Palliative Care): Deliver virtual palliative care including initial assessments, longitudinal panel management, symptom management, goals of care counseling, and hospice transitions for high-risk patients with life-limiting illnesses with an accent on compassionate, patient-centered care and interprofessional collaboration. Focus on evaluating hospital admissions, reducing readmissions, prognostication, and supporting seamless care transitions.

Location: Remote USA. Must possess current, active, unrestricted NP or PA license in at least one U.S. state and be willing to obtain additional state licenses as required.

Salary: $120K-155K base range plus performance-based bonus ($10K-$20K) for total comp $130K-170K

What you will do

• Perform initial assessments and develop Palliative Plans of Care for newly enrolling members.
• Manage a longitudinal panel of 40-50 high-risk patients via telephonic/video encounters, providing symptom management, goals of care discussions, and hospice transitions.
• Evaluate hospital admissions, support care transitions, identify readmission trends, and develop solutions to maintain readmission rates ≤15%.
• Participate in regular huddles, weekly interprofessional team meetings, monthly panel reviews, and host weekly office hours.
• Support team during growth or absences with phone lines, outreach, initial visits, and consult monitoring.
• Collaborate with interdepartmental teams and attend Devoted Medical meetings; provide rotating on-call weekend/holiday coverage.

Requirements

• Current, active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in at least one U.S. state; willing to obtain additional licenses (fees covered).
• 5+ years in advanced practice, with 3+ years in Palliative Care.
• ACHPN® credential or willingness to obtain within 6 months.
• Bilingual in English and Spanish.
• Willingness to flex between time zones; comfortable with technology, fast-paced work-from-home environment.
• Strong interpersonal/communication skills; experience in interprofessional care teams; self-starter.

Nice to have

• ELNEC® or EPEC® training.
• Prior ICU, oncology, or geriatrics nursing experience.
• Home-based palliative care, interprofessional team leadership, or program development experience.
