What you will do

• Provide telephonic care coordination and management to identified patient populations.
• Collaborate with interdisciplinary clinical teams to facilitate assessments, evaluations, and care planning.
• Initiate and follow up on specialty service referrals to ensure continuity of care.
• Develop personalized care plans to resolve barriers to care and engage social support systems.
• Provide telephonic counseling and teaching regarding health promotion and chronic disease management.
• Advocate for members to ensure their needs and choices are fully represented and supported.

Requirements

• Must be based in the USA.
• Fluent in English and Spanish, both orally and in writing.
• Current valid RN multi-state license.
• Bachelor’s degree (BSN) or higher from an accredited school of nursing.
• At least 3 years of post-licensure care management experience.
• Experience in geriatrics and chronic illness management.

Nice to have

• Case Management certification or completion of CCM certification within 1 year of employment.
• Knowledge of Medicare and Medicare Advantage Plans.
