What you will do
- Provide telephonic care coordination and management to identified patient populations.
- Collaborate with interdisciplinary clinical teams to facilitate assessments, evaluations, and care planning.
- Initiate and follow up on specialty service referrals to ensure continuity of care.
- Develop personalized care plans to resolve barriers to care and engage social support systems.
- Provide telephonic counseling and teaching regarding health promotion and chronic disease management.
- Advocate for members to ensure their needs and choices are fully represented and supported.
Requirements
- Must be based in the USA.
- Fluent in English and Spanish, both orally and in writing.
- Current valid RN multi-state license.
- Bachelor’s degree (BSN) or higher from an accredited school of nursing.
- At least 3 years of post-licensure care management experience.
- Experience in geriatrics and chronic illness management.
Nice to have
- Case Management certification or completion of CCM certification within 1 year of employment.
- Knowledge of Medicare and Medicare Advantage Plans.
