Compensation: $27.00 - $30.00 per hour

What you will do

• Conduct outreach and patient registration, including organizing community engagement and outbound calls.
• Provide comprehensive care navigation, including appointment logistics, prescription support, and referral coordination.
• Assess Social Determinants of Health (SDOH) needs and enroll patients in relevant support programs.
• Collaborate with multidisciplinary care teams to develop and implement individualized care plans.
• Represent the company at community events to build trust and facilitate health education sessions.
• Document all patient interactions and care team communications across EHR and internal tools.

Requirements

• Fluency (verbal and written) in English and Spanish.
• Must be authorized to work in the United States.
• Must possess a Community Health Worker certification OR demonstrate 2,000+ hours of CHW work in the past three years.
• 1-3 years of experience in healthcare or healthcare navigation within the community.
• 2-5 years of experience in community work, advocacy, engagement, or organizing.
• High school diploma or GED.

Nice to have

• Knowledge of Latino customs and cultural norms.
• Familiarity with Google Workspace and documentation in an EHR.
• Training in motivational interviewing.
• Strong existing ties to the local Latino community and knowledge of local SDOH resources.
