What you will do

• Implement ACO products and execute Practice Transformation Plans to meet quality and financial goals.
• Manage clinical processes including wellness visits and transitions of care.
• Train physicians and office staff on value-based care technologies and workflows.
• Build and maintain relationships with providers and healthcare executives to enhance performance metrics.
• Collect and relay practice feedback to management to drive product and process improvements.

Requirements

• Must be a resident of New Mexico or Arizona.
• College degree or 4-6 years of relevant medical practice management experience.
• 3 years of experience with Electronic Health Records (EHR) for clinical/practice management.
• Ability to travel up to 25% within the market.

Nice to have

• Strong public presentation and communication skills.
• Experience with process mapping and work-flow analysis tools.
