What you will do

• Conduct outbound calls and digital outreach (SMS, email) to drive new patient enrollments.
• Document all incoming referrals and interactions in systems of record with timely and accurate updates.
• Coordinate with health plan case managers and external healthcare providers to achieve enrollment goals.
• Manage inbound calls from potential patients and partners, addressing concerns and providing guidance.
• Achieve member enrollment goals based on company KPIs.
• Work with families to schedule appointments and follow-up care.

Requirements

• Bachelor's degree in social work, healthcare, or a related field.
• Experience working with various healthcare referral systems.
• Familiarity with working with patients assigned to Medicaid health plans.
• Excellent customer service and communication skills.
• Must be based in the United States and have a reliable internet connection for remote work.
