Focus on traveling to members’ homes or community locations, collaborating with interdisciplinary team under physician guidance, and preventing hospital readmissions.

Location: Tampa, FL (onsite field work, Mon-Fri 10am-6pm local time, reliable transportation required)

Salary: $24/hr - $30/hr + annual bonus

What you will do

• Travel to members’ homes or community locations to address acute episodic medical, behavioral, and social needs.
• Provide high-quality compassionate care as part of an integrated team including APPs, nurses, pharmacists, and therapists.
• Deliver therapeutic interventions under direct physician guidance and assist with urgent member needs.
• Conduct clinical intake visits to onboard new members.
• Engage post-discharge or ED patients to prevent readmissions and resolve root causes.

Requirements

• EMT-Paramedic with active Florida license and/or NREMT
• PALS, ACLS, BLS certifications required
• 3+ years field clinical experience
• Driver’s license, good driving record, reliable transportation
• Strong critical thinking, communication, collaboration skills
• Experience using multiple technology platforms

Nice to have

• Pediatric patient experience
• Non-traditional paramedic roles (critical care, primary care, tele-health, community paramedicine, ER, value-based care)
• Bilingual Spanish
