Location: Remote (USA). Must work in CST, MST, or PST time zones. An active and clear RN and APRN or Physician Assistant license in AL, AZ, GA, IL, OH and/or TX is required at time of hire.

Salary: $120k–$155k base, Total Compensation $130k–$170k

What you will do

• Conduct virtual cardiology and heart failure assessments, including interpreting labs, imaging, and ordering diagnostics.
• Formulate individualized treatment plans, managing medication and lifestyle modifications for cardiovascular conditions.
• Initiate and manage guideline-directed medical therapies (GDMT) for heart failure patients.
• Collaborate with an interdisciplinary team of physicians, pharmacists, social workers, and RN clinical guides.
• Provide clinical escalation support and advisory for specialty clinic staff.
• Utilize and provide feedback to improve the company's proprietary electronic health information platform.

Requirements

• Bilingual proficiency in English and Spanish is required for conducting clinical assessments.
• NP or PA with 5+ years of experience in cardiology clinical practice (2+ years in outpatient preferred).
• Active RN and APRN or PA licensure in Texas, Ohio, Arizona, Illinois, Georgia, or Alabama.
• Comfort with telehealth platforms, EMRs, and AI chat tools.
• Secure and private workspace with reliable internet.
• Willingness to obtain multistate licensure within 90 days of hire.

Nice to have

• Cardiovascular Nurse Practitioner Certification.
• Understanding of managed care, including STARS/HEDIS measures and clinical comorbidity coding.
