Location: Remote from United States only (CST timezone required, must physically be in the US)

Salary: $40–47 per hour

What you will do

• Provide proactive care and triage for clinical issues via frequent virtual touchpoints.
• Conduct comprehensive assessments of clinical, psychosocial, discharge, and financial needs; establish milestones and goals.
• Build rapport with patients and families, integrating cultural factors into care planning.
• Develop and communicate comprehensive care plans with physicians and interdisciplinary team.
• Coordinate transitions of care post-ED/IP/UC discharge and refer to specialists as needed.
• Document assessments, interventions, and ensure effective, timely care delivery.

Requirements

• Licensed RN in at least one US state with eligibility for others
• Bachelor’s in nursing from accredited university
• Pediatrics experience in outpatient, home health, complex care, PICU, or emergency medicine
• Bilingual Spanish required
• Demonstrate cultural competence and family-centered care

Nice to have

• Minimum 1 year care coordination or case management experience
• Familiarity with Medicaid regulations
• Value Based Care (VBC) experience
• Virtual care experience
