What you will do

• Build RN case management workflows for core programs like transitions of care and longitudinal management.
• Manage a personal patient panel to set clinical and operational standards.
• Oversee daily operations for CHW team, including performance management, hiring, onboarding, and development of RNs and care managers.
• Monitor team and patient performance metrics like engagement, readmissions, and care progression; drive continuous improvement using data.
• Serve as escalation point for complex cases and model clinical best practices.
• Embed NCQA compliance, provide clinical input on workflows, and integrate with product, data, and payer requirements.

Requirements

• Compact RN license required; BSN preferred.
• Certifications: ACM, CCM, or CMAC preferred.
• 5–8+ years case management or care coordination nursing experience.
• Prior experience managing teams or contracted staff in startup or high-growth environments.
• Experience in value-based care, population health, or managed care preferred.
• Strong operator comfortable in ambiguity as a working manager handling both clinical and people leadership.
