What you will do

• Conduct remote nursing assessments via phone and video visits covering heart failure, medications, and psychosocial factors.
• Provide tailored clinical education on disease processes, symptom monitoring, and lifestyle counseling.
• Develop and evaluate personalized care plans in partnership with members and caregivers.
• Collaborate with an interdisciplinary team of nurse practitioners, pharmacists, and social workers.
• Coordinate care with primary care providers and other internal clinical teams.
• Contribute to operational improvements and the development of a virtual Heart Center of Excellence.

Requirements

• Active multi-state Compact RN license required.
• Must be based in the Mountain (MT) or Pacific (PT) time zone.
• Minimum 4 years of RN experience, including at least 2 years in a non-ICU cardiac setting.
• Bilingual in English and Spanish (must be able to conduct assessments in Spanish).
• Strong verbal communication skills and ability to multitask in a fast-paced environment.
• Comfortable with technology including EMRs, messaging platforms, and AI-driven tools.

Nice to have

• Experience in telehealth or remote clinical roles.
• Knowledge of the Medicare Advantage landscape.
