TL;DR

Community Care Advocate: Connecting retail employees to community and company resources addressing social determinants of health, basic needs, financial, legal, and mental health support with an accent on referrals, consultations, and follow-ups. Focus on linking to local/national resources, navigating benefits, and tracking outcomes for high-impact assistance.

Location: Onsite Monday-Friday 8:30-5pm in Queens, NY zip codes 11239, 11354, and 11432.

Salary: $30-$34/hour

What you will do

  • Identify needs and refer employees to work-life, financial, legal, and basic needs resources (housing, food, transportation).
  • Provide one-on-one consultations, link to local/national resources, and support applications/follow-ups.
  • Collaborate with customer care teams for emergency funding and referrals.
  • Consult on mental health resources, workplace benefits, and treatment options like hirify.global or insurance.
  • Document metrics on involvement reasons, demographics, referrals, and outcomes.
  • Build relationships with customer contacts, promote services, and submit routine reports.

Requirements

  • Bachelor’s degree in a related field
  • Proficiency in Spanish
  • Experience working with diverse populations in social work.
  • Knowledge of connecting individuals to local and external resources.
  • Ability to balance multiple stakeholders, flexibility, and creativity.
  • Excellent listening, communication, and diversity/inclusion skills.
  • Motivated for high-touch service; tech-savvy with various platforms.
