What you will do
- Conduct research on attributed patients to identify healthcare needs and potential barriers to enrollment.
- Perform telephonic outreach to caregivers and healthcare professionals to facilitate enrollment and onboarding.
- Handle inbound calls from caregivers, health plan coordinators, and hospital discharge planners.
- Develop deep knowledge of the Supplemental Security Income (SSI) program to support eligible caregivers.
- Collaborate with Provider Engagement Specialists to conduct on-site outreach at PCP offices and residential facilities.
- Manage relationships with healthcare organizations to support the re-engagement of enrolled patients.
Requirements
- Must be based in the United States.
- 2+ years of healthcare experience in care coordination, community health, patient engagement, or sales.
- Proven success working within a centralized contact center environment.
- Bilingual Spanish required.
- Access to reliable transportation for field visits and reliable internet access.
- Ability to work autonomously and pivot quickly in a startup environment.

- Prior experience utilizing a CRM platform for documentation.
