What you will do

• Conduct follow-up calls with patients post-discharge in Mandarin/Cantonese/English for medication reconciliation and regimen understanding
• Provide education and adherence support on medications for heart health and diabetes
• Collect blood pressure data and support HEDIS goals
• Collaborate with pharmacists/providers to resolve medication issues and prevent readmissions
• Document interactions in hirify.global’s platform and triage high-risk patients
• Coordinate care across hospitals, clinics, pharmacies; assist with referrals/authorizations
• Manage calls, meet performance metrics, and participate in process improvements

Requirements

• Bachelor’s degree in Nursing (BSN) and active RN license in New York
• Bilingual proficiency in Mandarin and/or Cantonese and English (daily use)
• Prior case management experience and care coordination knowledge
• Strong medication management skills including reconciliation
• Excellent communication in English/Chinese; computer proficiency (Google Workspace, Mac, Slack, Five9)
• Ability to prioritize in fast-paced environment; reliability, detail-oriented
• 40 hours/week (Mon-Sat up to 7PM PT); metrics-driven
• Pass background check; live/eligible to work in US

• Pharmacy operations familiarity
• Certified Diabetes Educator (CDE)
