What you will do

• Guide members through online programs for diabetes, hypertension, GLP-1, and cholesterol.
• Deliver high-quality self-management education via a cutting-edge online platform.
• Monitor blood glucose, blood pressure, labs, and health maintenance appointments.
• Strategically counsel members using data science to inform outreach methods.
• Serve as a leader in cardiometabolic care within a growing digital coaching practice.

Requirements

• Fluent in Spanish.
• Active, unrestricted CDCES certification through the CBDCE.
• Active, unrestricted Registered Dietitian (RDN/RD) or Registered Nurse (RN) certification with 5+ years of experience.
• Minimum of 1 year professional experience as a CDCES or at least 3 years of experience in the field of diabetes.
• Bachelor’s degree.
• Knowledge of or experience with blood glucose interpretation utilizing CGM technology.

Nice to have

• At least 1 year of practice with patients with type 1 diabetes.
• Additional training in informatics or technology.
• Experience in digital health or telehealth.
