What you will do

• Build relationships with patients telephonically and virtually to identify social barriers and align resources.
• Collaborate with a multidisciplinary team of clinicians and coordinators to ensure cohesive, patient-centered care.
• Conduct screenings and create personalized action plans tailored to the specific needs of high-needs patients.
• Provide emotional support to caregivers and assist in navigating complex family and support issues.
• Facilitate critical conversations regarding end-of-life and long-term care planning.
• Coordinate care between the behavioral health team and monitor clinical outcomes.

Requirements

• Must be based in the USA.
• Master’s degree in social work and an active state license.
• Minimum of 3 years of experience in medical social work, case management, or care coordination.
• Ability to work effectively in a fast-paced remote environment.
