What you will do
- Conduct virtual primary care visits via telehealth, including comprehensive assessments and acute care interventions.
- Manage chronic conditions such as hypertension, diabetes, and COPD, developing individualized care plans.
- Order and interpret diagnostic tests and coordinate follow-up care with specialists or community resources.
- Manage medication prescriptions, titrations, and refills ensuring patient safety and adherence.
- Provide health coaching, lifestyle modification guidance, and patient education.
- Collaborate with multidisciplinary teams and provide feedback to improve the electronic health platform.
Requirements
- Must be based in the USA.
- Active Nurse Practitioner (APRN) license in good standing in required states or the ability to obtain licensure in multiple states within 90 days of hire.
- Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
- Minimum 2 years of experience in primary care, urgent care, or internal medicine.
- Minimum 1 year of experience working as a Nurse Practitioner in the D2Me program.
- Active BLS certification at time of hire and flexibility to align with patient local time zones (9:00 AM – 5:00 PM).
Nice to have
- Experience managing chronic conditions and acute illnesses via telehealth.
- Familiarity with value-based care models, population health management, and quality metrics (HEDIS, STAR ratings).
- Comfort with digital documentation and telemedicine platforms (EHRs).
