What you will do

• Plan and organize collaborative services, develop and administer individualized Plans of Care for up to a year.
• Maintain consistent client interactions via services distribution, phone calls, and one-on-one sessions.
• Partner with Florida Department of Corrections and community facilities to recruit and present the program.
• Assess client needs, monitor treatment plans, and make professional recommendations impacting reentry success.
• Collaborate with community agencies to connect clients to resources and follow up on referrals.
• Document all interactions and outcomes in the Efforts-to-Outcomes (ETO) data system.

Requirements

• Must work on-site in Tampa, FL
• Experience in case management, client support, and reentry programs.
• Skills in needs assessment, plan development, and progress monitoring.
• Ability to build relationships and collaborate with community partners.
• Proficiency in data entry and professional documentation.
