Salary: $26.00 - $27.00 per hour

What you will do

• Coordinate health care services, enrollment, and follow-up care for refugee clients in San Jose.
• Conduct health needs assessments and create personalized Plans of Care for a six-month service period.
• Perform initial mental health screenings for clients aged 14 and above.
• Assist clients in navigating complex healthcare services, including OB-GYN, dental, and mental health care.
• Maintain detailed case documentation in the Efforts to Outcomes (ETO) database.
• Guide and train volunteers and interns to support service delivery.

Requirements

• Must be located in or have access to San Jose, CA to perform office and field duties.
• Minimum 1 year of case management experience with vulnerable populations.
• Fluent English (spoken and written) required.
• Bachelor’s degree in social work, public health, or a related field preferred.
• Valid driver’s license and access to a personal, insured vehicle.

Nice to have

• Proficiency in Dari, Pashto, or Spanish.
• Background in health or mental health services.
