What you will do

• Conduct patient outreach, engagement, and enrollment using video messaging, consultations, and text-based communication.
• Deliver personalized education, coaching, and adherence support via asynchronous video tools.
• Perform video-based medication reconciliation and monitor patient administration of medications to assess for side effects.
• Create short-form educational and motivational video content for personalized patient engagement.
• Identify and address medical, behavioral, and social challenges, escalating cases to clinical review when necessary.
• Collaborate with provider offices, pharmacies, and cross-functional teams to refine clinical protocols and educational materials.

Requirements

• Registered Nurse (RN) license in good standing.
• Minimum 5 years of nursing experience.
• Minimum 5 years of experience in patient engagement, health education, or care coordination.
• Bilingual in Spanish.
• Must be eligible to work in the US; no visa sponsorship is available.
• Must have availability to work evenings and weekends.

Nice to have

• Experience in telehealth, community health, or digital health settings.
• Certification in health coaching.
• Knowledge of Directly Observed Therapy (DOT) models.
• Experience working with Medicaid and Medicare Dual Eligible (D-SNP) populations.
• Familiarity with Salesforce or other CRM platforms.
