Salary: $115,000 - $140,000 annually

What you will do
- Conduct primarily telehealth video visits with limited occasional in-home visits in your local area.
- Perform comprehensive assessments focused on prevention, chronic condition support, medication optimization, and health maintenance.
- Identify care gaps, coordinate with internal and external teams, and develop individualized whole-person care plans.
- Utilize AI-enabled workflows and technology to reduce admin burden and maximize member interaction.
- Communicate empathetically to build trust with members and caregivers.
Requirements
- Active NP or PA license in at least one specified US state (AL, AZ, AR, CO, FL, GA, HI, IL, IN, KY, MS, MO, NC, OH, PA, SC, TN, TX); willing to obtain additional within 90 days
- 3+ years outpatient clinical practice in primary care, family medicine, internal medicine, or geriatrics.
- Active BLS certification.
- Comfort with telehealth video visits and electronic documentation systems.
Nice to have
- Familiarity with managed care models like STARS/HEDIS and care gap identification.
- Experience with comprehensive/preventive visits for Medicare populations.
- Supporting older adults or those with complex medical/social needs.
