TL;DR

Telehealth Preventative Primary Care Advanced Practice Provider (NP/PA): Conducting comprehensive telehealth assessments and evidence-based care for older adults with an accent on prevention, chronic condition management, and care gap closure. Focus on coordinating with interdisciplinary teams, optimizing medications, and building member relationships in a tech-enabled environment.

Location: Remote USA; active NP/PA license required in at least one of: AL, AZ, AR, CO, FL, GA, HI, IL, IN, KY, MS, MO, NC, OH, PA, SC, TN, TX (must obtain additional licenses within 90 days)

Salary: $115,000 - $140,000 annually

What you will do

  • Conduct primarily telehealth video visits with limited occasional in-home visits in your local area.
  • Perform comprehensive assessments focused on prevention, chronic condition support, medication optimization, and health maintenance.
  • Identify care gaps, coordinate with internal and external teams, and develop individualized whole-person care plans.
  • Utilize AI-enabled workflows and technology to reduce admin burden and maximize member interaction.
  • Communicate empathetically to build trust with members and caregivers.

Requirements

  • Active NP or PA license in at least one specified US state (AL, AZ, AR, CO, FL, GA, HI, IL, IN, KY, MS, MO, NC, OH, PA, SC, TN, TX); willing to obtain additional within 90 days
  • 3+ years outpatient clinical practice in primary care, family medicine, internal medicine, or geriatrics.
  • Active BLS certification.
  • Comfort with telehealth video visits and electronic documentation systems.

Nice to have

  • Familiarity with managed care models like STARS/HEDIS and care gap identification.
  • Experience with comprehensive/preventive visits for Medicare populations.
  • Supporting older adults or those with complex medical/social needs.
