What you will do

• Diagnose, treat, and manage dementia and acute episodic illnesses through telehealth comprehensive and acute visits.
• Implement and monitor annual dementia care plans for patients and their caregivers.
• Collaborate with neurologists, interdisciplinary care teams, and community primary care physicians.
• Provide specialized dementia education and management skills to caregivers.
• Manage urgent calls and triage problems through on-call rotations during daytime hours.
• Maintain compliant documentation within the Electronic Medical Record (EMR) system.

Requirements

• Active, unrestricted APRN licensure and current Nurse Practitioner credentialing body certificate.
• Masters of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduation.
• At least 2 years of experience as an APRN in primary care, internal medicine, geriatric care, neurology, or equivalent.
• Strong working knowledge of Geriatrics and the care of Neurology patients.
• Proficiency with EMR, Slack, email, and other clinical documentation platforms.
• Ability to assess psychosocial needs in a culturally diverse environment.

Nice to have

• Experience as an APRN specifically in a gerontology and/or dementia care setting.
• Clinical research experience in AD/ADRD and knowledge of the current research landscape.
• Bilingual fluency in Spanish (verbal and written).
