Senior Care Manager (RN)

Location: Remote-NY; Monday-Friday, 8am-5pm EST

Salary: $75,300-$135,400 per year

What you will do

• Assess member needs and develop, monitor, and update personalized care plans and service plans.
• Coordinate care between members, families, caregivers, healthcare providers, specialists, and community resources.
• Identify care barriers, facilitate referrals and treatments, and revise interventions for complex or escalated cases.
• Educate members and caregivers about disease processes, care options, provider instructions, referrals, and healthcare benefits.
• Document care activities and member information in compliance with state, federal, and payer regulations.
• Coach and support clinical new hires and preceptees during onboarding and competency development.

Requirements

• NY RN state licensure or compact state licensure is required.
• Degree from an accredited school of nursing or a Bachelor’s degree in Nursing.
• 4-6 years of related experience.
• Ability to work Monday-Friday during EST business hours.
• Experience coordinating with members, caregivers, providers, and cross-functional teams.
