Care Manager (RN)

Location: Remote, Washington State. Working hours are Monday–Friday, 8:00 am–5:00 pm Pacific Standard Time.

Salary: $56,200–$101,000 per year.

What you will do

• Assess member needs, barriers to care, and social determinants of health.
• Develop and update personalized care plans and service plans with members, caregivers, providers, specialists, and community resources.
• Coordinate timely access to appropriate healthcare services and monitor member progress toward care goals.
• Provide outreach by phone, digitally, at home, or at other sites to assess needs and coordinate resources.
• Educate members and families about diseases, care gaps, provider instructions, care options, referrals, and healthcare benefits.
• Document care management activities and provide feedback on opportunities to improve care quality and cost-effectiveness.

Requirements

• Degree from an accredited school of nursing or a Bachelor’s degree in Nursing.
• 2–4 years of related experience.
• RN state licensure and/or Compact State Licensure required.
• Work from Washington State and maintain the required licensure.

Nice to have

• Experience working with maternal and infant populations.
• Experience supporting members in behavioral health crises.
