Registered Nurse Care Manager (RNCM)

Location: Riverside, California, United States. Hybrid work combining remote clinical duties with field-based visits and in-person member and provider engagement across Riverside, the Inland Empire, and San Bernardino.

Salary: $95,000–$110,000 per year.

What you will do

• Review member charts, medical histories, treatment adherence, and care plans in EMR/EHR systems.
• Provide clinical oversight and guidance to non-clinical Lead Care Managers handling complex cases.
• Identify gaps in care and recommend interventions to improve health outcomes.
• Coordinate referrals, care transitions, discharge planning, and follow-up with providers, hospitals, and community organizations.
• Conduct field-based visits, in-person member assessments, and engagement activities as needed.
• Participate in multidisciplinary case reviews and provide clinical input for regulatory and health-plan reporting.

Requirements

• Active Registered Nurse license in California required.
• At least 2 years of nursing experience.
• At least 1 year of experience in care management, case management, or a leadership capacity.
• Experience supporting vulnerable populations with complex medical and social needs.
• Strong clinical assessment, care planning, communication, collaboration, and clinical judgment skills.
• Ability and willingness to perform field-based work and in-person member engagement as required.
