Care Manager (RN)

Location: Remote in New Jersey with up to 50% local travel for in-home member visits. Central New Jersey candidates, particularly those in Essex, Hudson, Mercer, or Union County, are strongly preferred. Schedule is typically Monday-Friday, 8:00 AM-5:00 PM ET, with flexibility based on member and business needs.

Salary: $56,200-$101,000 per year

What you will do

• Assess member needs, barriers to care, and social determinants of health.
• Develop, coordinate, monitor, and update personalized care and service plans.
• Collaborate with members, families, caregivers, providers, specialists, and community resources.
• Provide care management interventions, outreach, referrals, and resource support.
• Educate members and caregivers about disease processes, care options, healthcare benefits, and provider instructions.
• Document care management activities and provide feedback on opportunities to improve care quality and cost-effectiveness.

Requirements

• Active New Jersey Registered Nurse (RN) license or compact-state RN licensure required.
• Degree from an accredited school of nursing or a bachelor's degree in nursing.
• 2-4 years of related experience.
• Availability for up to 50% local travel and in-home member visits is required.
• Spanish-speaking ability is strongly preferred.
