Salary: $20 - $25 per hour

What you will do
- Schedule patient visits in the EHR to optimize the time of RNs and APPs.
- Coordinate patient care in senior living communities and individual homes to drive clinical outcomes.
- Manage communications and relationships with patients, caregivers, families, providers, and pharmacies.
- Facilitate telehealth visits with virtual providers using tablets.
- Perform back-office support including fax management, data entry, and health plan insurance identification.
- Conduct direct resident visits to support ongoing care.
Requirements
- Must be based in or be able to travel within Chubbuck & Pocatello, ID.
- High School Diploma / GED.
- 2+ years of experience in healthcare with a focus on care coordination and administrative support.
- Medical Assistant, Patient Care Technician, or equivalent certification (or 5+ years of healthcare experience).
- Valid and current driver's license and comfort driving to various senior home settings.
- Proficiency in electronic medical record (EMR) documentation and general technology.
