Salary: $20 - $25 per hour

What you will do

• Schedule patient visits in the EHR to optimize the time of RNs and APPs.
• Coordinate patient care in senior living communities and individual homes to drive clinical outcomes.
• Manage communications and relationships with patients, caregivers, families, providers, and pharmacies.
• Facilitate telehealth visits with virtual providers using tablets.
• Perform back-office support including fax management, data entry, and health plan insurance identification.
• Conduct direct resident visits to support ongoing care.

Requirements

• Must be based in or be able to travel within Chubbuck & Pocatello, ID.
• High School Diploma / GED.
• 2+ years of experience in healthcare with a focus on care coordination and administrative support.
• Medical Assistant, Patient Care Technician, or equivalent certification (or 5+ years of healthcare experience).
• Valid and current driver's license and comfort driving to various senior home settings.
• Proficiency in electronic medical record (EMR) documentation and general technology.

Nice to have

• Experience in geriatric care or senior living communities.
