Resident Care Coordinator (Senior Living): Provide clinical and operations support to seniors in senior living communities and individual homes with an accent on care coordination, scheduling, and communication. Focus on optimizing RN and provider time, managing telehealth visits, and fostering relationships with caregivers, families, and community staff.
Pullman, WA. Travel within Lewiston / Moscow, ID & Pullman, WA. Must possess a valid driver's license and be comfortable driving to senior homes and communities. Job is 8am-5pm plus drive time.
Hourly Rate: $20 - $25

What you will do
- Schedule patient visits in EHR, coordinate with patients and caregivers.
- Handle inbound calls, texts, voicemails, faxes, data entry, and insurance management.
- Support RN and APP in care coordination, collaborate with community resources.
- Manage telehealth visits and conduct direct resident visits.
- Build relationships with patients, caregivers, families, providers, and senior living staff.
- Drive engagement through education and obtaining consents.
Requirements
- High School Diploma / GED (or higher)
- 2+ years healthcare experience in care coordination and admin support; Medical Assistant or equivalent certification preferred (or 5+ years experience)
- Experience in geriatric care or senior living highly desirable
- Valid driver's license and comfort driving to various senior settings
- Excellent EMR documentation and tech skills
- Ability to multitask, positive attitude, flexibility in startup environment
