What you will do

• Act as a liaison between clinicians and members to provide high-touch telehealth support and care coordination.
• Coordinate durable medical equipment (DME), skilled nursing services, and home health care with partners.
• Manage lab testing and imaging requests, diligently tracking results for clinician review and notifying members.
• Support pre-visit video onboarding to ensure members are comfortable with telehealth technology.
• Assist members in navigating the healthcare system and improving adherence to their specific plan of care.
• Handle medical record collection, information gathering, and appointment scheduling.

Requirements

• Must be based in the USA.
• MA Certification (highly preferred, expired certifications are acceptable).
• 2 years of patient-facing experience.
• Prior experience in primary care.
• Ability to work specific shifts in ET (EST/CST or MST/PST time zones).

Nice to have

• Experience with telehealth platforms.
• Background in Medicare care coordination and closing stars care gaps.
• Experience in Palliative/Hospice, Oncology, Dialysis, Behavioral Health, CHF, or Diabetes settings.
• Proficiency with electronic health records and Google Workspace (Chrome, Sheets).
