Salary: $85,000 - $95,000

What you will do
- Conduct assessments, check-ins, and in-person care for seniors in communities and homes to identify needs.
- Assist advanced practice providers in developing and implementing chronic condition care plans.
- Coordinate care with primary providers, specialists, families, and community staff for seamless delivery.
- Partner with internal team including resident care coordinators and virtual providers.
- Maintain documentation in EMR and triage after-hours calls.
- Provide clinical expertise to on-site staff and adapt to evolving startup processes.
Requirements
- Active compact Registered Nurse (RN) license in Washington or Idaho.
- Valid and current driver's license; comfortable driving to homes and communities.
- Strong background in geriatric/senior care, assessments, and coordination.
- Experience in chronic condition care planning and interdisciplinary collaboration.
- Excellent EMR documentation and technology proficiency.
- Ability to work autonomously, manage ambiguity, and adapt in a startup.
