What you will do
- Coordinate patient visits in the EHR to optimize the time of RNs and APPs while meeting patient needs.
- Manage inbound and outbound professional communications via calls, texts, and faxes.
- Maintain updated historical records and patient data within the EHR in an organized manner.
- Facilitate telehealth visits using tablets and conduct direct resident visits.
- Foster positive relationships with patients, families, pharmacies, and senior living community staff.
- Educate eligible residents and families on services to secure necessary consent forms.
Requirements
- High School Diploma / GED (or higher).
- 2+ years of experience in healthcare with a focus on care coordination and administrative support.
- Medical Assistant or Patient Care Technician certification preferred (otherwise 5+ years of healthcare experience).
- Valid and current driver's license for travel to various senior home settings and communities.
- Proficiency in electronic medical record (EMR) documentation and general comfort with healthcare technology.
- Experience in geriatric care or senior living community is highly desirable.
