What you will do

• Review daily caseload and prepare technology for patient services.
• Navigate schedule using modern tools and partner with in-home clinical team members (Sprinters).
• Conduct patient-centric wellness exams including health risk assessments, medication reviews, and cognitive screenings.
• Collaborate with Sprinters to evaluate vital signs, arrange blood draws, and address patient needs.
• Provide comprehensive care coordination, preventive interventions, specialist referrals, and documentation.

Requirements

• Board Certified as a Family Nurse Practitioner in California
• Active Family Nurse Practitioner License in California
• Pass national background check and valid clinical license search
• Technologically savvy with laptops/mobile devices for charting and HIPAA-secure messaging
• Strong written and verbal communication skills
• Ability to work independently or in a team
