Salary: $73,700 - $103,180 + equity + benefits

What you will do

• Deliver coordinated, patient-centered virtual care management by telephone and video to improve health outcomes.
• Create impactful care plans with members and diverse care team to achieve goals.
• Help members navigate complex medical conditions, treatment pathways, benefits, and healthcare system.
• Partner with local providers for coordinated care and provide compassionate longitudinal follow-up.
• Assist during acute episodes like hospitalizations and rehabilitation with case management support.
• Coordinate clinical and social resources to holistically address members' needs.

Requirements

• Bachelor of Science in Nursing (BSN).
• Current CCM Certification or eligibility within 6 months of hire.
• 5+ years of nursing experience; 2+ years in care management.
• Active Compact RN license in good standing; willingness to obtain licensure in multiple states.
• Comfortable discussing various medical conditions across age ranges.
• Experience working remotely and using multiple computer/medical record systems.
• Empathetic, efficient in clinical history gathering, patient-level communication, and HIPAA compliance.
