What you will do

• Review caseload and prepare technology for the day.
• Navigate schedule using modern tools and partner with in-home clinical team members (Sprinters).
• Build connections with diverse patients while collecting information and performing services.
• Conduct patient-centric wellness exams including health risk assessments, medication reviews, and cognitive screenings.
• Collaborate with Sprinters to evaluate vital signs, arrange blood draws, and address patient needs.
• Provide care coordination including preventive interventions, medication management, specialist referrals, and documentation.

Requirements

• Board Certified as a Family Nurse Practitioner in California
• Active Family Nurse Practitioner License in California
• Exhibit highest levels of professionalism, integrity, accountability, confidentiality, care, and compassion.
• Willingness to work in a revolutionary environment requiring last-minute problem-solving and innovative thinking.
• Technologically savvy with laptops/mobile devices for charting and HIPAA-secure messaging.
• Strong written and verbal communication skills; ability to work independently or in a team.
• Pass national background check and valid clinical license search.
