What you will do

• Provide comprehensive longitudinal care integrating primary and behavioral health to improve measurable patient outcomes.
• Manage acute and chronic conditions to reduce avoidable emergency department visits and hospitalizations.
• Conduct behavioral health screening, diagnosis, and front-line management for anxiety, depression, ADHD, and substance use.
• Prescribe and manage medical and psychotropic medications following evidence-based guidelines.
• Utilize data-driven tools (PHQ-9, GAD-7) to guide clinical decisions and track patient progress.
• Collaborate with a multidisciplinary team of specialists and care coordinators to ensure coordinated care.

Requirements

• Active, unrestricted medical license in at least 40 U.S. states (50 preferred).
• Board Certification in Family Medicine or Internal Medicine.
• Experience in integrated care delivery and managing common behavioral health conditions.
• Strong understanding of value-based care principles, quality metrics, and utilization management.
• Proficiency with telemedicine platforms and virtual care delivery.
• Availability to work 40 hours per week, including evenings, weekends, and occasional overnights.

Nice to have

• Experience in Medicare Advantage, Medicaid, ACOs, or employer-sponsored populations.
• Familiarity with HEDIS and STAR ratings.
• Training in addiction medicine or collaborative care models (CoCM).
• Multi-state licensure via IMLCC.
• Prior experience in a digital health or virtual-first organization.
