Salary: $115,000 - $120,000/year

What you will do
- Act as primary point of contact for people-management inquiries across a 75+ employee org supporting multiple service lines.
- Drive operational performance in inbound/outbound teams, care gap closure, referrals, follow-ups, and digital workflows.
- Develop scalable, data-driven performance frameworks for virtual care programs including primary care and disease management.
- Direct supervisors and frontline staff, advance care coordination workflows with clinical and operational leads.
- Monitor KPIs, guide training and chart auditing for compliant operations.
- Collaborate with vendors and partners to improve workflows and the clinical management platform.
- Mentor staff, foster collaboration, and support career growth through coaching.
Requirements
- Working Schedule: 9am-5pm EST
- Bachelor's Degree (preferred) in Healthcare Administration, Nursing, Human Resources, or similar.
- 3+ years managing people with accountability for quality and productivity.
- Demonstrated ability to drive new initiatives and achieve business outcomes.
