What you will do

• Evaluate members, establish physical status, and develop individualized physical therapy programs.
• Collaborate with primary care and specialty providers to optimize care plans and member experience.
• Conduct telehealth visits to support and promote expanded access to care.
• Document therapeutic treatments and evaluate patient progress.
• Coordinate with other disciplines to meet individual member needs.
• Identify and participate in the resolution of member safety issues.

Requirements

• Doctor of Physical Therapy (DPT) degree from an APTA-accredited program.
• Current applicable state temporary/permanent license as a Physical Therapist.
• BLS certification (current or obtained within 30 days of hire).
• Must be based in or able to work on-site in Brunswick, Maine.
• 3+ years of experience in a hospital, long-term care, or outpatient setting preferred.
• Analytical ability to conduct thorough evaluations and develop sound treatment plans.
